Day of Your Exam, what you need to bring

Please bring the following items with you:

Government issued photo ID: passport, state or government ID, or US Driver License
Refugees: I-94 card
Vaccination records (if any)
You may eat and drink prior to your examination.
I-693 form: we will assist you in completing the form
